29 CFR  1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  (a) On or about June 15, 2022 at the construction site located at N74W17963 Mineral Road, Menomonee Fall, WI. Employees were performing framing work on a residential home without fall protection. Exposing employees to approximately 10 feet 11 inches fall hazard.

29 CFR  1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   (a) On or about May 31, 2022, at the jobsite located at N74W17963 Mineral Road, Menomonee Falls, WI 53219, the employer did not ensure that employees exposed to potential fall hazards had been trained on fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) On or about June 15, 2022 at the construction site located at N74W17963 Mineral Road, Menomonee Fall, WI, employees accessing the roof to conduct carpentry work from a ladder which did not extend at least three feet above the roof.
