29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 16, 2022 an employee was exposed to a struck-by falling debris hazard working on lower ground level beneath a occupied scaffold, where masonry work was being performed.

29 CFR  1926.102(a)(2):The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  On or about June 16, 2022 an employee was exposed to eye damage hazard by using a DeWalt electric saw to cut stone/brick without wearing eye protection.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  On or about June 16, 2022 an employee was exposed to a fall hazard while performing masonry work on the 2nd level of a scaffold approximately 14 feet above lower ground level without fall protection.
