29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At a worksite located at 960 Cedar Ridge, Raymore, MO, three employees were exposed to fall hazards greater than six feet. The employer exposed the employees to this hazard by allowing them to work at heights greater than 6 feet without the use of fall protection or mitigation measures.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a worksite located at 960 Cedar Ridge Dr, Raymore, MO, employees were exposed to fall hazards. The employer exposed employees to these fall hazards by allowing them to access the roof of the structure by using a portable ladder that was not extended 3 feet or more above the upper landing surface for which it was being used to access.
