29 CFR  1926.28(a): The employer did not ensure that appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:  a) Ecua Home Improvement LLC. - 640/642 Convery Blvd., Perth Amboy NJ 08861  The employer did not ensure employees engaged in work from a scaffold at various elevations and below were not wearing PPE to task; no hard hats, safety glasses, gloves or Personal fall arrest. In not doing so exposed employees to fall hazards, struck by falling debris or tools, eye hazards and laceration hazards.  Condition noted on 17 June 2022
