Storage of material created a hazard:   On or about June 22, 2022, and at times prior thereto, the employer did not ensure that flexible intermediate bulk containers (FIBC) in the warehouse were stacked so that they were stable and secure to prevent the bags from collapsing. Employees were exposed to the hazards of being crushed-by and/or struck-by, associated with collapsing flexible intermediate bulk containers (FIBC).
