29 CFR 1926.451(b)(1):Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights.   On or about June 24, 2022, at job site 38 Norfolk Ave., Clarendon Hills, IL.  60514, the employer failed to ensure that scaffolds were fully planked, employees were exposed to fall hazards in excess of 15 feet while standing on a metal scaffold engaged in construction activities.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section   On or about June 24, 2022, at job site 38 Norfolk Ave., Clarendon Hills, IL.  60514, Employees were exposed to fall hazards in excess 15 feet height while performing construction activities from a metal scaffold. No form of fall protection was being utilized.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On or about June 24, 2022, at job site 38 Norfolk Ave., Clarendon Hills, IL.  60514, Employees used the frame of the scaffold to climb down, therefore exposing employees to falls of 15 feet. The employees were not provided a safe means of access to the working area of the scaffold.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.   On or about June 24, 2022, at job site 38 Norfolk Ave., Clarendon Hills, IL.  60514, employees performing masonry  work at a residential property were exposed to fall hazards approximately 15 feet with no fall protection when the employer failed to ensure they were adequately trained to recognize and minimize the hazards associated with fall.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
