29 CFR 1910.1048(d)(1)(iii): Sampling measurements used to determine an employee's exposure were not representative of the employee's full shift exposure to formaldehyde:    Location: Howard K. Hill Funeral Services: Embalming Room  Embalmers who were monitored on 10/24/2011 for exposure to formaldehyde were not monitored correctly, as the pump's flow rate was set at 0.1 liters per minute (lpm) rather than the approved flow rate of 0.2 lpm when determining the short term exposure limit (STEL) for formaldehyde exposure.
