29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  On or about June 09, 2022, employees were exposed to struck by hazards while performing roofing activities on a residential home located at 3455 Birch Lane Lot 29 Fairlawn, Ohio 44333. The employee was using a air powered nail gun and the employer did not ensure that they were using eye protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 06/09/2022, employees were exposed to a fall of approximately 10 feet, while performing roofing activity on a residential home located at 3455 Birch Lane Lot 29 Fairlawn, Ohio, 44333. The employees were working on all areas of the roof and the employer did not ensure they were using any form of fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about June 09, 2022, employees were exposed to a fall hazard of approximately 10 feet, while performing roofing activities on a residential home located at 3455 Birch Lane Lot 29 Fairlawn, Ohio, 44333. The employees using an extension ladder to access the roof and the employer did not ensure that it extended at least 3 feet above the landing area.
