29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a- Roof Of 42, 44, 46, 48, 50 Autumn Park Drive, Medford NJ: Workers engaged in roof shingle placement, installation, and other related activities were exposed to fall hazards in excess of 30 feet to the ground surface below when walking on and working from a residential structure's pitched roof without the use of fall protection.  Condition noted  on or about 06/28/22.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)   a- Roof Of 42, 44, 46, 48, 50 Autumn Park Drive, Medford NJ: Workers engaged in roof shingle placement, installation, and other related activities were exposed to fall hazards in excess of 30 feet to the ground surface below when walking on and working from a residential structure's pitched roof without the use of fall protection. These workers had not received fall protection training as specified in 29 CFR 1926.503 (a)(2)(i) through (viii). Condition noted  on or about 06/28/22.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a- Roof Of 42, 44, 46, 48, 50 Autumn Park Drive, Medford NJ:  Workers were exposed to fall hazards, up to and in excess of 30 feet to the ground surface below, when using an aluminum extension ladder whose side rails did not extend at least 3 feet above a landing surface being accessed.  Condition noted  on or about 06/28/22.
