29 CFR  1910.22(a)(1):All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  Establishment-  The workplace floor was not kept in a clean and orderly fashion as a result of cutting boards and several puddles that were on the floor exposing employees to trip hazards.

29 CFR  1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the working space did not permit at least a 90-degree opening of equipment doors or hinged panels:  Establishment: Live panels of 220 volts were being blocked from opening 90 degrees by various equipment, in mounts, in front of  the panels.
