29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) In the production area: On or about June 22, 2022, the employer exposed employees to amputation hazards, in that, the employer allowed employees to remove the guard on the crimping machine.  b)  In the production area: On or about June 22, 2022, the employer exposed employees to amputation hazards, in that, the employer allowed employees to remove the guard on the foot pedal that activated the crimping machine.
