29 CFR 1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about June 30, 2022, the employer did not ensure procedures were utilized for the control of potentially hazardous electrical and pneumatic energy of the Plant 2, 75-Liter Tilt Mixer Machine, when employees were engaged in service and maintenance activities, such as but not limited to, cleaning material from the mixing chamber.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(l) through (d)(6): On or about June 30, 2022, the employer did not ensure the operator(s) of the Plant 2, 75-Liter Tilt Mixer Machine performing service and maintenance activities applied and followed the established energy control procedures by; turning off or shutting down the machine [per 1910.147(d)(2)], isolating the machine from the electrical and pneumatic energy source(s) [(d)(3)], affixing lockout device(s) to the energy isolation devices [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to cleaning material from the machine's mixing chamber.
