29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about July 1, 2022, the employer did not ensure that employees utilized or complied with the lockout/tagout procedures for Royal 4 Extruder (#4852). Employees were exposed to struck by and/or caught in between hazards involving the moving auger screw when moving the existing, unaffixed guarding to access the screw without deenergizing or locking out the equipment.

Employees are exposed via a 11"x9" opening that is 9" away from the screw.

29 CFR  1910.212(a)(2):Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  On or about July 1, 2022, the employer failed to ensure that Royle 4  Extruder (Serial #4852) was effectively guarded to protect employees from coming in to contact with the moving auger screw in that the existing guarding was hinged at the bottom and not secured at the top, allowing for employees to open the guarding and access the auger screw.
