On or about July 5, 2022, employees were exposed to struck-by hazards from modifications added to a Caterpillar loader, without the manufacturer's written approval.

On or about July 5, 2022, the employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions while constructing high fences:  (A) The employer did not provide employees gloves when working with barb-wire fencing.  (B) The employer did not provide employees safety glasses while working in and around hammer post driver.
