29 CFR  1910.332(b)(1):Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  a) Hord Personnel Services. on or about July 7, 2022, at the site address of 1059 Township Rd 1806 Ashland, OH: The employer did not ensure an employee operating a 36ft. long conductive trailer mounted boom auger, was trained in and familiar with the safety-related work practices when working near energized overhead powerlines. A feed driver while operating the boom auger from the trailers control panel was exposed to electrocution when boom was raised in close proximity of a 7.2K powerline.

29 CFR  1910.333(c)(3)(iii)(A):Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained:  a) Hord Personnel Services. on or about July 7, 2022, at the site address of 1059 Township Rd 1806 Ashland, OH: The employer did not ensure an employee operating a 36ft. long conductive trailer mounted boom auger, was kept a safe distance from energized overhead powerlines. A feed driver while operating the boom auger from the trailers control panel was exposed to electrocution when boom was raised in close proximity of a 7.2K powerline.
