On or before July 7, 2022, at a worksite located at 5435 Maple Drive East, Lot 9099, Lewis Center, Ohio 43035: The employer failed to ensure employees used electrical equipment where the path to ground was permanent and continuous. Employees used an extension cord that was missing the ground prong, thereby being exposed to a 110-volt electrical contact hazard.   b. On or before July 7, 2022, at a worksite located at 5435 Maple Drive East, Lot 9099, Lewis Center, Ohio 43035: The employer failed to ensure employees used electrical equipment where the path to ground was permanent and continuous. Employees used an air compressor with a cord that was missing the ground prong, thereby being exposed to a 110-volt electrical contact hazard.   c. On or before July 7, 2022, at a worksite located at 5435 Maple Drive East, Lot 9099, Lewis Center, Ohio 43035: The employer failed to ensure employees used electrical equipment where the path to ground was permanent and continuous. Employees used a 5-Outlet Extension Cord Hub that was missing the ground prong, thereby being exposed to a 110-volt electrical contact hazard.

On or before July 7, 2022, at a worksite located at 5435 Maple Drive East, Lot 9099, Lewis Center, Ohio 43035: The employer failed to ensure the scaffold platform was adequately attached to a telescopic rough terrain forklift and the forklift was not moved horizontally. Employees used a job-made platform on a telescopic rough terrain forklift where the platform was tied to the rack with nylon straps and the forklift was moved horizontally while the employee was on the platform.

Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  a. On or before July 7, 2022 at a worksite located at 5435 Maple Drive East, Lot 9099, Lewis Center, Ohio 43035: The employer failed to ensure employees who worked at heights of 6 feet (1.8 m) or more above lower levels were protected from falls. An employee was framing the roof of a two-story house without any means of fall protection, thereby exposed to fall hazards up to 30 feet.
