29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a)OA Roofing at 4400 South Grinnell Avenue, Sioux Falls, SD 57106:  On or about and at times prior to July 11, 2022, the exposing employer did not ensure frequent and regular inspections of the job site were being conducted.  Employees were exposed to fall hazards due to a lack of fall protection systems as well as ladder hazards identified and corrected on the site.  This condition exposed employees to fall hazards greater than 12 feet.

This condition exposed employees to fall hazards associated with improper use of employer provided ladders.
