29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   a) Facility wide - On or about July 11, 2022, the employer did not ensure that employees who operate powered industrial trucks, including Toyota 9BRU18 electric stand up rider trucks, Toyota 8BPU15 order picker trucks, Toyota 8FBE20U electric sit down rider trucks, the Hyster H50FT LPG sit down rider truck, and the Yale ESC030FAN36TE094 electric stand up rider truck, were properly trained, evaluated and certified to perform tasks in a safe manner, thereby exposing employees to struck-by hazards.
