29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: a. In the warehouse, on or about June 30, 2022, and at times prior, employees were exposed to struck by hazard. Employees were not provided refresher training after being involved in forklift incidents, near misses and other unsafe acts. The employees were required to continue to operate the forklift without being retrained in the relevant topic.
