29 CFR  1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a)  250 North Middlesex Road Elementary School, Front Side - Two employees were working from  a JLG 660SJ  Mobile Elevated Working Platform  while removing mortar from block joints, elevated approximately 8 to 10 feet above a lower level, while wearing personal fall arrest equipment but not attached to the anchorage points, on or about July 13, 2022.   (b)  250 North Middlesex Road Elementary School, Back Side - Two employees were working from  a JLG 660SJ  Mobile Elevated Working Platform  while power washing the block, elevated approximately 15 to 16 feet above a lower level, while wearing personal fall arrest equipment but not attached to the anchorage points, on or about July 13, 2022.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
