The employees were working from a horse scaffold on top of a  supported scaffold and the employer did not ensure the working level was fully planked.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.   On or about July 13, 2022, employees on the ground level were exposed to a struck by hazard while performing masonry activities on a residential home located at 1662 North Eckert, Akron, Ohio 44313.  The employees were working from the top level of a supported scaffold with tools and materials and the employer did not ensure they installed toe boards to prevent them from falling.
