29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   At a worksite located at 206 NW Plaza Dr, Riverside, Missouri. Employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the  all forklift operators had not completed  a competent training program. Including but not limited to the Caterpillar Forklift E5000 Serial # A4E0240873.

29 CFR  1910.178(m)(3): Unauthorized personnel were permitted to ride on industrial truck(s)  At a worksite located at 206 NW Plaza Dr, Riverside, Missouri. Employees were exposed to serious, fall, caught-in, crushing and or struck-by hazards, in that a forklift operator permitted a rider on the forklift known to the company as Caterpillar E5000 forklift, Serial # A4EC240873 with a carpet attachment. The carpet attachment was used to place and retrieve rolls of carpet from a warehouse storage rack approximately 16 feet in height.
