29 CFR  1910.303(b)(1):Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:   (a) Kitchen area, including tray line and adjacent dish room:   On or prior to 7/14/22, 120v and 208v receptacles were not provided with ground fault circuit interrupter (GFCI) protection, exposing employees to shock hazards while working in the area.

29 CFR  1910.303(b)(7)(iv):  There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating.  (a) Kitchen area, including tray line and adjacent dish room:   Circuit breaker boxes, disconnect switches, and associated conduit and with bent and/or corroded covers were in use, exposing employees to electric shock hazards.

29 CFR  1904.41(a)(2): Employer with establishment having 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and establishment is classified in an industry listed in appendix A to subpart E of this part, did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee:  (a) St. Francis at the Knolls, Inc. located at 20 Grand Street, Warwick, NY 10990:  On or prior to 7/25/2022, the employer did not electronically submit information from their  OSHA Form 300A or equivalent  for calendar year 2021 by March 2, 2022.  The establishment employed 60 employees during calendar year 2021  and is classified 623312 during calendar year 2021.  Note: Employer must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.
