29 CFR  1926.651(c):Means of egress from trench excavations.  A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.:  a)  Grant Village Camp Ground, Yellowstone National Park:  On or about July 14, 2022, and at times prior, employees were exposed to entrapment hazards while installing underground utilities in a unprotected trench measuring 8 feet in depth without safe means of access and egress.  The HK Contractors, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 1491596, citation number 1, item number 4, and was affirmed as a final order on March 18, 2022, with respect to a workplace located at 302 Robert Reamer Avenue, Gardiner, MT, 59030 .

29 CFR  1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  a)  Grant Village Campground, Yellowstone National Park:  On or about July 14, 2022, and at times prior, employees were exposed to struck by hazards while working inside a trench with a spoil pile that was not set back at least 2 feet from the edge of the trench.    HK Contractors Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.651(j)(2), which was contained in OSHA inspection number 1491596, citation number 1, item number 3, and was affirmed as a final order on March 18, 2022, with respect to a workplace located at 302 Robert Reamer Avenue, Gardiner, MT, 59030 .

HK Contractors Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.651(k)(1), which was contained in OSHA inspection number 1491596, citation number 1, item number 2a, and was affirmed as a final order on March 18, 2022, with respect to a workplace located at 302 Robert Reamer Avenue, Gardiner, MT, 59030.
