29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.   a. Fortress Framing Inc. at 6107 E 141st Lane, Thornton, CO 80602: On July 16, 2022, the exposing employer did not ensure that employees of NYTO Builders LLC., subcontractor, were protected from injury, in that portable ladders, used to access the roof of a new residential home had side rails that did not extend at least 3 feet above the upper landing or was secured. The employer did not protect employees against falls at heights over 20 feet. This condition exposes to employees to fall hazards.

Fortress Framing Inc. at 6107 E 141st Lane, Thornton, CO 80602: On July 16, 2022, the exposing employer did not ensure that employees of NYTO Builders LLC., subcontractor, were protected from injury, in that portable ladders, used to access the roof of a new residential home was used on a stable and level surface to prevent accidental displacement. The employer did not protect employees against falls at heights over 20 feet. This condition exposes to employees to fall hazards.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a. Fortress Framing Inc. at 6107 E 141st Lane, Thornton, CO 80602: On July 16, 2022, the exposing employer did not ensure that employees of NYTO Builders LLC., subcontractor, were protected from injury, in that employees were observed conducting framing operations at heights over 20 feet on a new residential home building without the use of fall protection. The employees were exposed to fall hazards.  The Fortress Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1408921, citation number 1, item number 1 and was affirmed as a final order on July 2, 2019, with respect to a workplace located at 11591 26th Ave., Aurora, CO 80010.
