29 CFR 1910.28(b)(3)(i):  The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):  a) Facility, 550 Sunnyside Road, Suite 101, Bedford, PA:  On or about June 17, 2022,  an employee was exposed to a fall hazard while on the roof of the Defiance Metal Products building working on the air conditioning unit that cooled the Powder Coat EV room below, in close proximity to a 106" x 70" unprotected skylight.
