(b) On and preceding July 18, 2022, the crew employed by Jairo Bona at 5757 Pennyflower Court, Westerville, OH 43081 were observed dismounting trusses to use an interior ladder to exit the under-construction building, adjacent to two uncovered missing sections of flooring measuring approximately 4' by 3' each, exposing them to an interior fall hazard measuring approximately 9'.

The side rails of the ladder did not extend at least 3' above the upper landing surface being accessed, which exposed employees to fall hazards.
