(a) On and preceding July 18, 2022, the crew employed by Jairo Bona at 5757 Pennyflower Court, Westerville, OH 43081 were observed  working at heights exceeding 6' without the use of hazard awareness and response programs.  The on-site supervisor did not give training to the employees in fall protection, nor did he enforce the use of PPE on the job site, which exposed the employees to fall and struck-by hazards.

(b) On and preceding July 18, 2022, the crew employed by Jairo Bona at 5757 Pennyflower Court, Westerville, OH 43081 were observed dismounting trusses to use an interior ladder to exit the under-construction building, adjacent to two uncovered missing sections of flooring measuring approximately 4' by 3' each, exposing them to an interior fall hazard measuring approximately 9'.

The crew used a section of an extension ladder to access the first floor by separating part of the ladder for independent use, a purpose other than which the ladder was designed.

The side rails of the ladder did not extend at least 3' above the upper landing surface being accessed, which exposed employees to fall hazards.
