29 CFR  1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection.  Location - 585 Chandler Street, Tewksbury, MA.  On or about July 20, 2022, an employee was work/walking on a site made scaffold platform raised by an all-terrain  powered industrial truck to a height of eleven feet with no fall protection. The scaffold platform had no railings on it nor did the employee have fall protection system in place to prevent him from falling.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Location - 585 Chandler Street, Tewksbury. MA.  On or about July 20, 2022, two employees were exposed to fall hazards greater than nine feet above lower level. The employees were working/walking at or near unprotected window openings.

On or about July 20, 2022, employer did not provide a stairway or ladder for an access point with an elevation greater than nineteen inches above lower level.
