29 CFR  1904.41(a)(1): The employer did not electronically submit information from the three recordkeeping forms that they keep under this part (OSHA Form 300A Summary of Work-Related Injuries and Illnesses, OSHA Form 300 Log of Work-Related Injuries and Illnesses, and OSHA Form 301 Injury and Illness Incident Report) to OSHA or OSHA's designee:  a) On or about July 20, 2022, the employer failed to electronically submit the information from their OSHA Form 300A or equivalent by March 2, 2022, for calendar year 2021. The establishment employed over 250 employees  during calendar year 2021.
