29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   a) Jobsite, Pittsburgh Plaza East, 1700 Lincoln Highway,  North Versailles, PA : On or July 20, 2022, A ladder, ramp or any other safe means of egress were not located in the trench excavation.  Employees working at the bottom of a trench had to walk on loose trench debris to the shallow end of the excavation to exit.

Employees working at the bottom of a trench that were exposed to struck-by hazards in the event of materials falling or rolling into the excavation.

29 CFR  1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems:    a) Jobsite, Pittsburgh Plaza East, 1700 Lincoln Highway,  North Versailles, PA : On or July 20, 2022,  The foreman acting as the competent person did not conduct daily inspections of the excavation that would have required the usage of cave-in protections when the depth of the excavation reached 48 inches.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) Jobsite, Pittsburgh Plaza East, 1700 Lincoln Highway,  North Versailles, PA : On or July 20, 2022, during excavation activities where a 12-inch diameter corrugated pipe was installed, the employer did not ensure that an adequate cave in protective system was in place, in there was no cave in protection inside the trench that was measured to be approximatly 7 feet 5 inches in depth were an employee was working.
