29 CFR 1926.102(a)(1): Eye and face protective equipment being used did not used meet the requirements specified in Z 87.1-1968:    The employer is failing to protect employees from struck by hazards. This was most recently documented on July 21, 2022 at a residential construction jobsite located at 1600 11th Street, Beatrice, Nebraska. Employees were operating pneumatic nail guns while shingling a roof without the use of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards. This was most recently documented on July 21, 2022 at a residential construction jobsite located at 1600 11th Street, Beatrice, Nebraska when employees were observed working on a residential structure with a roof pitch greater than 4:12. The employees where installing shingles while working at a height of approximately 14 feet 2 inches measured from the ground to eave without fall protection.

29 CFR 1926.1053(b)(1): The employer is exposing employees to fall hazards where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards. This was most recently documented on July 21, 2022 at a residential construction jobsite located at 1600 11th Street, Beatrice, Nebraska when employees were observed working on the roof of a residential structure. The ladder used by the employees was not extended over the upper landing surface.
