Combustible waste materials, such as oily rags, were left stored near flammable fluids at the service bay area. This condition exposed the employees to serious hazards while handling flammable liquids and combustible materials.

An eye wash station was not provided for the employees to use for quick drenching or flushing of the eyes from the corrosive chemicals. This condition exposed employees to eye injuries while handling corrosive chemicals.

The work rest of the bench grinder had an approximately one-half inch opening from the abrasive wheel. This condition exposed employees to struck-by hazards while using the bench grinder.

The adjustable tongue guard of the bench grinder was approximately two inches from the abrasive wheel. This condition exposed employees to struck-by hazards while using the bench grinder.
