29 CFR  1926.304(d):  Portable, power-driven circular saw(s) were not equipped with a guard below the base plate or shoe:  a.  At 619 Ponte Vedra Boulevard, in Ponte Vedra Beach, Florida:  On or about July 26, 2022, the employer exposed employees to amputations, cuts, lacerations, and abrasions, in that the guard on a portable saw was not utilized while operating a portable saw to cut siding which was being installed on the front of the newly constructed commercial building.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a.  At 619 Ponte Vedra Boulevard, Ponte Vedra Beach, Florida:  On or about July 26, 2022, the employer exposed an employee to fall hazards ranging from 6 to 41-feet, in that fall protection, such as, a body belt with a lanyard attached to the boom or basket  was not utilized, while operating an aerial lift JLG, model 450 AJ,  to cut and  install siding on the front of the the newly constructed commercial building.  b.  At 619 Ponte Vedra Boulevard, Ponte Vedra Beach, Florida:  On or about July 26, 2022, the employer exposed employees to fall hazards ranging from 6 to 41-feet, in that fall protection, such as, a body belt with a lanyard attached to the boom or basket  was not utilized, while operating an aerial lift SkyJack, model SJ63AJ to cut and  install siding on the front of the newly constructed commercial building.

29 CFR  1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a.  At 619 Ponte Vedra Boulevard, Ponte Vedra Beach, Florida:  On or about July 26, 2022, the employer exposed employees to fall hazards ranging from 6 to 41-feet, in that fall protection training  was not provided prior to these employees operating an aerial lift to cut and install siding on the front of the newly constructed commercial building.
