29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) An employee was installing roofing on new construction without a suitable means of fall protection. The roof was estimated 5:12 to 6:12 pitch with a height at the peak near 18 feet.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface, the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) An employee used an unsecured metal utility ladder to access a roof for to install roofing on new construction. The top rung of the ladder was even with the eave and no grasping device was present. NOTE: Certification of corrective action (abatement) is required for this item.
