29 CFR  1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a. Roof Area, at the Pavilion located across the street from Building 365 in Mayport Naval Station, Florida 32228; On or about July 27, 2022, the employer exposed employees to fall hazards, in that fall protection was not utilized by employees while performing ventilation training to cut holes into the roof.  Employees were being exposed up to a 12-foot fall hazard.
