29 CFR  1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a) Warehouse, 40 Executive Avenue, Edison NJ 08817.  Employee(s) operate powered industrial trucks including electric reach trucks and pallet jacks without having received instruction and evaluation on their safe operation, as documented through written certification.  Condition(s) were noted on or about 7/27/2022.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  a) Warehouse of 40 Executive Ave., Edison, NJ 08817  The employer failed to certify that each operator had been trained by their previous employer. The employer failed to provide documentation which included the name of the operator, the date of evaluation and the identity of the person performing the training  Condition(s) were noted on or about 7/27/2022.  House of Spices (India) Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1360785, citation number 1, item number 5 and was affirmed as a final order on 4/10/2019, with respect to a workplace located at 40 Northfield Avenue,  Edison NJ 08837.
