29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about July 26, 2022, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a.  On or about July 26, 2022, at the above addressed jobsite,  employees conducting roofing operations, twenty (20) feet  nine (9) inches above the ground, were not protected from falling.  b.  On or about July 26, 2022, at the above addressed jobsite,  employees conducting roofing operations, nineteen (19) feet six (6) inches above the ground, were not protected from falling.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about July 26, 2022, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
