29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On July 25, 2022 and times prior thereto, at a work site located in Arlington, Texas, an employee installing sewage pipe in an unprotected excavation that was approximately 19 feet long x 5 feet depth x 2 feet wide.  The employee was exposed to cave-in hazards.
