29 CFR  1910.178(n)(1):  All traffic regulations were not observed, including authorized plant speed limits, and/or a safe distance was not maintained as required and/or the truck was not kept under control at all times.  Location: Warehouse, 415 Wildwood Avenue, Woburn, MA.  a) On or about 7/29/22, employees operating powered industrial trucks did not maintain a safe distance between trucks, or kept the trucks under control at all times. An incident occurred where one reach-type powered industrial truck struck another reach-type powered industrial truck.
