29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 2, 2022, in the Maintenance Machine Shop area, employees were operating a TOS Trencin Manual Lathe, while sanding on a unguarded rotating component parts of a auger shaft.

29 CFR 1904.39(a)(2):The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On or about July 26, 2022, an employee was sanding a shaft on a lathe machine when his gloved hand was pulled into the rotating part machine, the employee was hospitalized without the employer reporting the incident to OSHA within twenty-four (24) hours.
