29 CFR  1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:   Employee(s) engaged in residential framing at a residential construction site located at 10121 S. 191st Ave, Omaha, Nebraska, were exposed to fall hazards in that a platform unit attached to a powered industrial truck being used by employee(s) was not secured.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Employee(s) engaged in residential construction framing activities at a residential construction site located at 10121 S. 191st Ave, Omaha, Nebraska, were exposed to fall hazards in that employee(s)  were engaged in work on a roof of about 10ft 9inches with a pitch of about 9/12 without fall protection.
