29 CFR  1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a) Warehouse Area- On or about August 1, 2022, the employer did not ensure that powered industrial truck(s), such as the Clark Forklift #10, that were equipped with front end attachments other than the factory installed attachments, such as the Cascade Push Pull Attachment ATPP02-034, were marked to identify the attachments and show the approximate weight of the truck and the attachment combination at the maximum elevation with the load laterally centered.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
