29 CFR  1926.451(b)(2): Each scaffold platform and/or walkway was not at least 18 inches wide:  a) Jobsite located at or near 708 Latona St, Philadelphia, PA - On or about July 27, 2022, the employer did not ensure that scaffold platforms and walkways were at least 18 inches wide. Employees were pouring concrete from side bracket scaffold platforms attached to cast in-place concrete forms that were less than 18 inches wide, thereby exposing employees to fall hazards.

29 CFR  1926.451(e)(5)(i): Ramps and walkways 6 feet or more above lower levels did not have guardrail systems that comply with subpart M of this part -- Fall Protection:  a) Jobsite located at or near 708 Latona St, Philadelphia, PA - On or about July 27, 2022, the employer did not ensure that ramps and walkways 6 feet or more above lower levels had guardrail systems. Employees were utilizing wooden planks greater than six feet from the next lower level, without a guardrail, to access cast in-place concrete forms, thereby exposing employees to fall hazards.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Jobsite located at or near 708 Latona St, Philadelphia, PA - On or about July 27, 2022, the employer did not ensure employees exposed to fall hazards 6 feet or more above the ground were protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees were standing on top of cast in-place concrete forms without the use of fall protection, thereby exposing employees to falls greater than six feet from the ground.
