29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system:  The employer is failing to protect employees from fall hazards. This was most recently documented on August 4, 2022 at 1700 15th Street, Beatrice, Nebraska when employees were observed working on a residential structure with a roof pitch less than 4:12. The employees where installing shingles while working at a height of approximately 9 feet 11 inches measured from the ground to eave without fall protection.

29 CFR 1926.1053(b)(1): The employer is exposing employees to fall hazards where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards. This was most recently documented on August 4, 2022 at 1700 15th Street, Beatrice, Nebraska when employees were observed working on the roof of a residential structure. The ladder used by the employees was not extended over the upper landing surface.
