Two employees were responsible for wave soldering activities which involved the use of a tin/lead alloy which contained 25-50% lead were potentially exposed to lead at or above the action level.

29 CFR  1910.1025(l)(1)(i):Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the contents of Appendices A and B of 29 CFR 1910.1025.  Location: 10 Controls Drive, Shelton, CT 06484 - On or about August 4, 2022, employees who operate the wave soldering machine utilizing an alloy of 50-75% tin and 25-50% lead were potentially exposed to airborne lead.  The employer did not provide them with lead training including the contents of the Appendices A and B of 29 CFR 1910.1025.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  10 Controls Drive, Shelton, CT 06484:  On or about August 4, 2022, those employees responsible for the wave soldering process line were not trained in the hazards associated with lead.  The alloy used in the process contained 25-50% lead.

29 CFR  1910.1200(f)(6)(ii):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.   Location: 10 Controls Drive, Shelton, CT 06484 - On or about August 4, 2022, the label on a spray bottle of Alpha 857 Flux WS, a Category 2 flammable liquid containing 70-80% isopropyl alcohol, which was used in the wave soldering process was not labeled in accordance with GHS requirements in that it did not have pictograms or warnings indicating the hazards of the chemical.

29 CFR  1910.1025(m)(1)(iii): The employer did not include lead in the hazard communication program established to comply with the HCS (� 1910.1200), and the employer did not ensure that each employee had access to labels on containers of lead and to safety data sheets, and that employees were trained on lead in accordance with the requirements of HCS and paragraph (l) of this section.   10 Controls Drive, Shelton, CT 06484 - On or about August 4, 2022, the employer's written Hazard Communication Program did not include the hazards associated with lead and employees were not provided with training in accordance with paragraph (l) of this section.  Employees who performed wave soldering of printed circuit boards used a heated alloy containing 25-50% lead.

COMPANY-WIDE:  On or about August 4, 2022, the employer's written Hazard Communication Program did not include an inventory of hazardous chemicals that were used at the facility.  Employees regularly used chemicals including, but not limited to, an alloy of tin and lead and isopropyl alcohol.
