29 CFR 1926.451(c)(1):Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph: a) 2500 South Abilene Street, Aurora, CO 80012: On or about August 5, 2022, as a creating, exposing and correcting employer, Fox Valley Construction Group, Inc., exposed employees to fall hazards while working on a scaffold with a height-to-base of more than 4:1 ratio. The scaffold was not being properly restrained from tipping by guying, tying, bracing or its equivalent.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a)2500 South Abilene Street, Aurora, CO 80012: On or about August 5, 2022, as a creating, exposing and correcting employer, Fox Valley Construction Group, Inc., did not ensure that employees conducting work from a scaffold were properly trained by a qualified person to recognize hazards associated with the scaffold such as the lack of bracing or tying of the scaffold to prevent tipping. This exposed employees to fall hazards.
