29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about August 10, 2022, an employee working in and around a trench was not protected by a hard hat, exposing the employee to struck-by hazards.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  On or about August 10, 2022, an employee was exposed to struck-by hazards as excavated material not kept at least two feet from the trench edge.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
