29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Top Line Commercial Roofing, LLC. on or about August 10, 2022, at the address of 795 Berdan Rd. Toledo, OH: The employer did not ensure employees were protected from falls while performing roofing activities atop a steep sloped commercial type of structure. Employees installing roofing material from atop the structure were exposed to falls over 18 ft. 8" to the blacktop lot below.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Top Line Commercial Roofing, LLC. on or about August 10, 2022, at the address of 795 Berdan Rd. Toledo, OH: The employer did not ensure employees had been trained in appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing activities. Employees in this condition were exposed to falls over 18' 8" to the black top surface below.
