29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. Commercial worksite, at 2205 Haines Street, (Jacksonville Firemen's Credit Union), in Jacksonville, Florida 32220: On or about August 9, 2022, the controlling employer Forshee Contracting Services, LLC failed to ensure its sub-contractor HYR Roofing, Inc. protected its employees from eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roofing shingles.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:  a.  Commercial worksite, at 2205 Haines Street, (Jacksonville Firemen's Credit Union), in Jacksonville, Florida 32220: On or about August 9, 2022, the controlling employer Forshee Contracting Services, LLC failed to ensure its sub-contractor HYR Roofing, Inc. protected its employees from fall hazards ranging from 9 feet 7 inches to 16-foot, 5-inches, in that fall protection was not utilized by employees while installing roof shingles on a 4:12 pitched roof.
