OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushed by  and struck by hazards created by unsafe utilization of racking systems:  (a) 400 Cheryl Ave, Mechanicsburg PA, South wall of the warehouse - On or about August 12, 2022, bin D011A in the racking system against the South wall of the warehouse had visible damage to the horizontal cross beam exposing employees to struck by hazards.  (b) 400 Cheryl Ave, Mechanicsburg PA, South wall of the warehouse  - On or about August 12, 2022, bin D011A in the racking system against the South wall of the warehouse did not have the load limit labeled on the racking system exposing employees to struck by hazards.  (c) 400 Cheryl Ave, Mechanicsburg PA, South wall of the warehouse - On or about August 12, 2022, bin D011A in the racking system against the South wall of the warehouse was not anchored nor centered at the vertical support beam which was pushed out of alignment after being struck by a powered industrial truck exposing  employees to struck by hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  (a) 400 Cheryl Ave, Mechanicsburg PA, South wall of the warehouse - Employees operated power industrial trucks in racking  where the horizontal members can enter the operator compartment exposing them to crushing hazards.   The employer did not ensure that the facility was evaluated and employees were protected from an underride hazard, on or about August 3, 2022.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
